Dr. Baker has practiced Physical Medicine and Pain Management in Lebanon / middle Tennessee since 1999.


T. Scott Baker, M.D.

Dr. Baker has practiced in Lebanon, Tennessee since July, 1999, 22+ years. He is a non-surgical specialist in acute and chronic musculoskeletal pain, interventional pain management, and electrodiagnsotic medicine. His primary board certification is the American Board of Physical Medicine and Rehabilitation, and he has board certifications from American Board of Pain Medicine and the American Board of Electrodiagnostic Medicine.

Dr. Baker is currently the president of the Tennessee Academy of Physical Medicine and Rehabilitation (2021-2023). He is the secretary for the Tennessee Pain Society’s Board of Directors. He has served on the Tennessee Department of Health’s Chronic Pain Guideline Committee since 2015. Dr. Baker is a member of the Tennessee Department of Labor’s Medical Impairment Registry (MIR) program.

Dr. Baker is a Cum Laude graduate in Biology from the University of Louisville in 1990, and graduated from University of Louisville’s School of Medicine in 1995. Dr. Baker then went onto specialize in Physical Medicine and Rehabilitation completing a four year residency at The Ohio State University Medical Center. OSU Medical Center’s rehabilitation program has been recognized as among the best rehabilitation programs in the country in the latest “America’s Best Hospitals” issue of U.S. News & World Report magazine. Our program has consistently ranked in the top 10 of the Best Hospitals listing in the specialty of rehabilitation and has made the list for 13 consecutive years. Dr. Baker served as Chief Resident in 1998/1999.

When Dr. Baker is not in the office, he is spending time with his family (wife Jean Ellen, and 3 grown children), or exercising: being an avid tennis player, he also enjoys CrossFit and Skiing. He and his wife are active members of Providence United Methodist Church.
